You're leaving out the MOST important thing - rape isn't really about sex, it's about power. As I already noted, in cultures where masculinity is not equated with domination and control of women, rape functionally doesn't exist.

Poor sex education and shame no doubt play a part, but that isn't what has taught boys that they ought to keep pushing past a woman's No in order to get what they want.
